Orthopedics Plus will be offering FREE sports physicals for any Russellville middle school or high school athlete who plans on participating on any athletic sports team next school year on Tuesday, June 14, from 4-8 p.m.
The physicals will take place at the Orthopedics Plus Clinic, 105 Robins Way # 201, Russellville.
There will be time slots for each grade level to attend. If the time slot doesn't work for any students, please contact Coach Calvin Head to schedule another time slot.
All students must have pages 1-3 completed in full before the physical can occur. Links to the forms are shown below. Physical forms can be picked up in the front offices at RMS and RHS.
Time slots are as follows: 4-5 p.m. Incoming 4th-6th graders; 5-6 Incoming 7th and 8th graders; 6-7 Incoming 9th and 10th graders; 7-8 Incoming 11th and 12th graders.
